trouble with xyron 250

Hello, on 12/26/10, my sister had trouble with her xyron 250. She was using a permanent adhesive refill cartridge. She had these problems:

• Adhesive on her leg.
• Material getting flipped over, therefore having adhesive applied on the wrong side. 
• Crank getting stuck  and top separating from the rest of the machine.
• Adhesive applied on the right side of the material, but not enough.

Can you tell me how to solve these problems,

why they are happening, and how to keep them from reoccurring.
